The Cwm Taf Morgannwg (CTM) region is an area spanned by the University Health Board of the same name and covering the council areas of Rhondda Cynon Taf, Bridgend and Merthyr Tydfil. Its Regional Partnership Board (RPB), which is a group consisting of a range of public and third sector organisations in this area, is working on health and social care responses to increasing population need.
In our region there is an increasing number of people living into older age, which is something to be celebrated, and a trend in which over many years health and social care services have played a positive role in enabling. However, more people living longer with complex health conditions inevitably leads to increasing pressure on health and social care services in the region.
A key priority for the region, in response to these pressures, is the implementation of the CTM Model of Integrated Community Care Services. The model consists of two pathways. Firstly, Population Health Management, which will utilise linked data to develop proactive responses to people’s emerging health needs. Secondly, Urgent Community Response, which will provide an appropriate rapid response in the community to an urgent health and/or care need. The aim here is that more people can avoid transit to an Emergency Department and admission to a hospital ward, if better alternatives that can treat and care for that person in their own home are available more consistently.
Implementing these pathways means working with existing community-based health and social care teams, to establish more effective structures and ways-of-working across the region – specifically integrated care teams based on a Cluster footprint. The Integrated Community Care program has been established to do this, working closely with local Clusters, the CTM Navigation hub and local authority and other health board strategic change initiatives.
In this context, Regional Integration Managers support the ambitions of the Regional Partnership Board (RPB) in driving transformational by promoting an integrated approach with system partners to deliver a sustainable health and social care service for the future. Being a Regional Integration Manager is a hands-on role, working directly with colleagues in strategic and particularly in front-line roles to actively re-shape service towards the CTM model for integrated community services. Over the coming months the future ‘to be’ model will be defined in detail, and ahead of winter 2024/25 the Regional Integration Manager will have made progress towards introducing the new model in their designated area, and go on to enable further change in the time ahead.
The post holder will manage dedicated project resources, and undertake program and project management responsibilities, often working within tight timescales and competing demands. This work will often involve working with complex and potentially contentious information with a high degree of autonomy.
The two posts being advertised will focus respectively on the Rhondda Cynon Taf and Merthyr Tydfil Council areas.
